Bronze Medal Plan
Qualifications:
A referral is required by your athletic club or dance studio. Each plan is for 12 months and can be renewed each year. If cancelled before the end of 12 months, the balance owed on functional testing and custom supplement 3 month cycle is required. Available only for ages 12 and up. Late fee of 10% automatically applies for declined transactions. Cannot be combined with any other special or coupon. Once cancelled cannot renew in same calendar year.
